Registration forms

These forms give us essential details regarding you and your health - it is important to fill them in as accurately and completely as possible - we may need to contact you if there are any omissions or errors. We also need consent to text and/or e-mail you if needed.

Please always give us your Birmingham address including postcode.

You do need to complete and print the consent to text form and the alcohol form enclosed in the second download if you are an adult. If you score more than 4 on the alcohol form then you must attend the practice to see a nurse before your registration is complete.

You may either come into the surgery or post the forms to us to complete your registration.
