Pre-Registration

Come and register with us....

We are often very busy at the start of the University year with large numbers of students wanting to register with us. By spending the next few minutes pre-registering, you can save time by completing most of the process on-line.

To complete your registration you must come into the surgery or post the forms

Please remember that we can only register you if you live in our practice area - so check here if you are uncertain.

Please try and complete each of the following screens as fully as you can. This may not be possible if you are new to the country or do not know your NHS number. Even if you cannot fully complete a form, you are still able to pre-register but note that these forms are only for NHS registration and we may have to check that you are eligible to receive NHS services.
