Change of Address
Please note that we have a student address page that shows the exact information that we require for student halls of accommodation.
Please also note that if your new address is outside our practice area, then we will be unable to accept the address and will write to you.
If you have moved a short way outside our area , then South Birmingham Primary Care Trust will be able to advise you of alternative practices that cover where you live - contact them on 0121 687 4600 or click here.
Inform us of any changes
The best way to inform us of your new address is to use this link to EMIS - our computer software suppliers - via their encrypted server.
Please note that before you use this service for the first time, you have to visit the surgery to obtain your unique practice ID number and password. Our practice ID is 6044.
