Introduction

Under the Freedom of Information Act 2000 all public authorities are required to have and operate a publication scheme approved by the Information Commissioner. Doctors providing medical services under most contracts with the NHS in England are public authorities in respect of information relating to those services. This Publication Scheme complies with the suggested model publication for General Practices published by the Information Commissioner.


How is the Information Made Available?
The information within each Class is made available either on this website and/or in hard copy upon application to the Practice Manager.

Your Rights to Information

The Freedom of Information Act 2000 recognises that members of the public have the right to know how public services are organised and run, how much they cost and how decisions are made. It obliges us to respond to requests about information that we hold, in any format, within 20 working days; it creates a right of access to many but not all items of information. These rights are subject to some exemptions, which have to be taken into consideration before deciding what information will be released. The main reasons to decline release of information are the protection of commercial interests and personal information and this applies to all Classes within the Publication Scheme. Data that is only available in a raw form and needs manual or computer searches to identify may also not be available.

Under the Data Protection Act 1988, you are also entitled to access your clinical records and any other personal information held about you - to do this you need to contact the Practice Manager and make an appointment to visit the practice. You cannot ask for individual information on another person.


How much will it Cost?
Whilst the publication is free of charge there may be a cost to produce copies of the surgeries policies and procedures. These costs are our standard photocopying charges(the current rate for photocopying and/or printing is 40pence per sheet and the administrative charge is £10.00.) The charges will be reviewed regularly. The publication is available, free of charge, on the practice website:
 

Classes of Information

All information at the University Medical Practice is managed in accordance with NHS guidelines and the Data Protection Act.. Our commitment to publish information excludes any information, which can be legitimately withheld under the exemptions set out in the Freedom of Information Act 2000.
